The WHO surgical checklist has existed for a decade and everyone says they follow it. We audited 3,258 surgeries against the three checklist moments and found the sign-out —the final count before closing— was completed in just 26%. A process nobody verifies is a suggestion; now real adherence is measured, case by case.
The dashboard said 114% of target; the real margin —after discounts— was 92%, with dozens of products selling below cost and nobody seeing it. Today the dashboard shows the real money.
